HealthPathways offers clinicians locally agreed information to support decision-making at the point of care. It integrates clinical guidance, service and referral options, and patient resources into a single platform tailored for your local context. It can help students, junior doctors, registrars, IMGs (international medical graduates) and GPs know ‘how things are done around here’.
Community HealthPathways is developed by GPs and primary care clinicians for general practice teams. Local GP or primary care clinical editors collaborate with specialists, and health system and community stakeholders to regularly update pathways to ensure they are practical, reflect local reality and are evidence-based. It provides structured guidance for consultations, refining differential diagnosis, managing rare conditions, curated resources, and insights into the local health system.
HealthPathways is free to use for health professionals. It is available across almost all of Australia. It is funded and supported by State Departments of Health, Primary Health Networks, local Hospital health systems or a partnership of the three. The HealthPathways platform is delivered by Streamliners, a purpose driven organisation who partner with a network of members across Australia, New Zealand, the UK and Canada. Some regions also use hospital-specific pathways, Hospital HealthPathways.
